Initial Psychiatric Evaluation, adults ages 18 - 64:
75 min $400
Initial Psychiatric Evaluation for ADHD, adolescents ages 12-17:
90 min $450
QbTest Results Interpretation and Treatment Planning visit: $300
Standard Follow-up Visit:
20-30 min $240
Extended Follow-up Visit:
50-60 min $350
Weekly psychotherapy:
45 min $250
TMS Treatments: Generally covered by insurance. Individual costs may vary based on specific insurance plans and any relevant deductibles to be met.
Fees
Payment is due at each visit. We accept cash, checks, debit/credit cards. (Credit/debit card must be on file before an appointment is made).
A discount for follow-up visits is applied to private pay patients when paid at time of visit.
If paperwork is required for any reason you should schedule an appointment and have it completed in session. If you ask to have paperwork completed or a letter written outside of a session a $50-$100 fee based on time will be assessed.
Insurance
Dr. Dunkin accepts PPO for Blue Cross Blue Shield, Aetna, Cigna, United/Optum at this time.
Currently not taking Medicare patients.
A super bill is available through the portal that can be submitted to other insurance for reimbursement if you have out-of-network benefits and have met your deductible.
Payment is due at the time of service. Checks, cash, and credit/debit cards are accepted at in-office visits. For tele-psychiatry visits, a credit/debit card will be charged.
Cancellations and Missed Appointments
Your appointment is reserved exclusively for you, so please give at least 48 business hours notice for any appointment which you are unable to keep.
Full fees are charged for missed appointments and cancellations with less than 48 business hours notice. Please note that insurance does not reimburse any portion of these missed appointment fees.
Missing appointments interferes with the efficacy and continuity of your care. A pattern of repeatedly missing appointments is grounds for termination of care.
Practice Agreement
Policies are covered in detail on the new patient intake forms. Completion of these forms is required prior to your first visit with Dr. Dunkin
Controlled medications
Strictly monitored under the DEA.
These medications are only filled during an appointment.
Dr. Dunkin does not start controlled medications by request or from any previous diagnoses or treatment plan. These medications are only started if it is determined that an individual is a good fit.
For your safety, Dr. Dunkin does NOT start controlled medications for those with an active substance use disorder.
Random drug screens are required for those using controlled medications.
